To begin the enrollment process you can print out the
Application Form.
Complete the form and mail it to Montessori Center
of South Dayton, Inc., Attn.: Admissions, 2900 Acosta Street, Kettering, OH 45420-3467.
Also enclose a $50.00 check made payable to "MCSD" for the Application Fee.
Upon receipt of the application we will contact you to schedule a personal tour
of our facilities, schedule a child evaluation with a teacher and answer any questions
you may have. You will also be given additional forms consisting of an Emergency
Medical Authorization form, Emergency Transportation form, Child’s Medical Statement,
Basic Field Trip Permission form, a Child Pick Up Authorization Form and a School
Supply form. All forms must be completed
and returned as soon as possible. In addition, a check in the
amount of $150.00 for the Materials Fee and a current photograph of your child(ren)
are required. Failure to submit all of the above forms and payments will result
in the delay of your child starting school.
All children that attend morning classes and need afternoon child care are escorted
from their respective classroom to the Child Care Center by a staff member. During
the school year, child care arrangements must be made in advance. Monthly calendars
are provided for scheduling purposes. It should be noted that the Child Care Center
is closed from 8:30 a.m. to 11:00 a.m.
